Percutaneous electrosurgical technique for treatment of subclavian vein occlusion: Application of transcaval techniques

چکیده
Introduction Occlusion of the subclavian vein is a relatively common complication following permanent pacemaker implants, with an incidence of up to 10%. This rarely manifests clinically, but it becomes a problem when upgrading pacemakers with additional leads. As the population ages the need to reaccess the occluded veins for cardiac resynchronization therapy or defibrillator leads is increasing. These occlusions can be frequently crossed with hydrophilic wires; however, there is no standard for treatment of wire-resistant lesions. Many of these patients will undergo contralateral device implants and risky laser lead extractions or epicardial leads. We present a safe, novel strategy for crossing chronically occluded subclavian veins with existing pacemaker leads using an angled stiff-tipped 0.014-in wire charged with electrocautery, enabling successful coronary sinus lead implant.

Chronic Total Occlusion of the Innominate Trunk: Successful Recanalization with Retrograde Technique and Dedicated Devices Borrowed from Percutaneous Coronary Interventions
The innominate trunk is a short vessel with crucial clinical relevance as in most patients it provides flow to the right subclavian artery and the right common carotid artery. Innominate trunk occlusions are particularly challenging, as results of endovascular therapy are suboptimal in terms of acute success, whereas open surgery poses a high risk of complications. The systematic application of...
متن کاملImplant of subcutaneous central venous access devices in outpatient surgery
Today, an increasing number of subcutaneous central venous access devices are implanted on a ambulatory basis either by percutaneous vein puncture or venous cut down. The aim of the present study was to prospectively evaluate which is the most suitable implant technique for ambulatory surgery by comparing subclavian vein puncture using a Seldinger technique with cephalic vein cut-down in terms ...
متن کاملPercutaneous transfemoral repositioning of malpositioned central venous access device: a report of two cases.
Placement of long term central venous access devices (CVAD) such as chemo ports and Hickman's catheters are associated with a definite risk of catheter tip malpositioning. As such, malpositioning runs a risk of venous thrombosis and related complications; it is imperative to reposition the catheter. Percutaneous transfemoral venous approach has been described as a minimally invasive and safe me...
متن کاملFailed surgical ligation of the proximal left subclavian artery during hybrid thoracic endovascular aortic repair successfully managed by percutaneous plug or coil occlusion: a report of 3 cases
Open surgical rerouting and proximal ligation of one or more supra-aortic vessels prior to endovascular stent-graft placement has become an alternative to major open thoracic surgery in the treatment of complex thoracic aortic disease. Complications owing to failed surgical ligation of the left subclavian artery are rare. In this report, 3 cases of failed ligation are presented. Diagnosis was m...
متن کاملEndovascular repair of latrogenic subclavian artery perforations using the Hemobahn stent-graft.
PURPOSE To report the use of a new self-expanding endograft for percutaneous treatment of iatrogenic subclavian artery perforations. CASE REPORTS The subclavian artery of 2 patients was inadvertently cannulated during percutaneous attempts to implant a permanent pacemaker in one and catheterize the subclavian vein in the other. Because both patients had serious comorbidities, endovascular rep...
